The survey of 810 women aged 18 or older showed 77.5% said "cosmetic surgery is necessary" to improve their looks.
It was conducted by a Kyung Hee University student as part of her doctoral thesis and quoted by Yonhap news agency.
Only 2.1% of those surveyed said cosmetic surgery is unnecessary while 20.4% disapproved of such surgery.
Those who said they had already received cosmetic surgery accounted for 47.3%, and the rate was 61.5% for women between 25 and 29.
The survey also showed some 70 reported feeling stress because of perceived defects in their appearance in a society which places a premium on good looks.
Plastic surgery is booming in South Korea as many women seek what they consider to be facial improvements to find boyfriends or better jobs.
The government recently announced new rules to discourage bosses from demanding photos in advance from female job applicants.
